HARAPAN

The HARAPAN (Human Resources for Advancing Private-sector Health Access and Patient Assistance Network) project, implemented by YKI with support from the World Bank and the Bill Melinda Gates Foundation, provided technical assistance to strengthen Indonesia’s innovative financing for TB programme. In close collaboration with the Ministry of Health, BPJS Kesehatan (BPJS-K), and District Health Offices, HARAPAN enhanced the capacity of 222 private primary care providers in Bogor, Semarang, and Surabaya to integrate more effectively into the National TB Programme (NTP).

The intervention focused on six strategic areas: case detection and management, diagnostic networks, recording and reporting, logistics, monitoring and evaluation, and demand generation. Through mentoring, on-the-job training, and development of standard operating procedures (SOPs), HARAPAN strengthened private providers’ ability to screen, diagnose, and manage TB cases while improving data accuracy in the Sistem Informasi Tuberkulosis (SITB). The project also facilitated provider access to BPJS-K incentive claims for TB services and supported public outreach through digital campaigns and e-poster dissemination to encourage care-seeking behaviour.

Key Achievements:

  • Average monthly drug-sensitive TB (DS-TB) notifications increased from 13 to 28 cases across supported providers.
  • Treatment initiation rose from 3 to 19 cases/month, while referral cases decreased from 9 to 5 per month, indicating improved service readiness.
  • Diagnosis incentive claims grew from 8 to 40 per month. In Semarang, diagnostic claims reached 86% of target; while in Surabaya, intensive treatment claims rose to 21% of target.
  • Strengthened collaboration between private providers and the public sector, contributing to more sustainable TB financing and improved service coverage.
