Yasa Karsa Insani (YKI), in collaboration with the Ministry of Health, implemented the AKSES VL (Accelerating Demand-making and Supply-side Strengthening for Optimal Access to HIV Viral Load Testing) project to expand equitable access to HIV viral load (VL) testing among people living with HIV (PLHIV) receiving antiretroviral therapy (ART). The initiative addressed both demand-side and supply-side barriers, combining community engagement, capacity building, logistics coordination, and laboratory quality improvement to strengthen Indonesia’s HIV diagnostic ecosystem.

On the demand side, the project mobilised communities through 34 provincial workshops, 100+ peer supporter groups, and 18 Instagram Live sessions featuring 31 key opinion leaders, reaching over two million people nationwide. On the supply side, Yasa Karsa Insani (YKI) improved the VL testing by training over 9,000 participants on HIV VL testing, expanding the SITRUST HIV system for real-time specimen tracking, and distributing 153,000 laboratory consumable packages. Participation in External Quality Assurance Systems (EQAS) increased from 112 laboratories in 2022 to 193 in 2023, supported by the national handover of the SITRUST HIV EQAS module.

  • To strengthen equitable access to HIV VL testing by enhancing community demand generation, provider capacity, specimen transport systems, and laboratory quality assurance across Indonesia.

Key Achievements:

  • Increased HIV VL testing coverage from 27% in 2022 (43,690 PLHIV) to 63% in 2023 (116,880 PLHIV), with viral suppression rates exceeding 90%.
  • Enhanced laboratory network efficiency, with 2,443 CST facilities and 4,125 transport routes actively utilising the SITRUST HIV platform.
  • Expanded EQAS participation to 193 laboratories, significantly improving quality control and reliability.
  • Distributed over 7,000 IEC materials to 2,043 CST facilities and 85 community organisations to promote VL testing and adherence.
  • Improved specimen turnaround time with 85% of samples processed within five days through expanded GeneXpert use.
